1.       Current State of Oklahoma employees in the classified service or those having reinstatement rights should submit a copy of all of the following application materials:

a. A cover letter containing the title and announcement number of the position for which you are applying. This information should include a telephone number where you can be reached during office hours, your mailing address, email address and your current classification and code.
and
b. a completed Office of Personnel Management Personal Data Summary Sheet, form OPM-4B. The OPM-4B must clearly indicate how you meet the education and/or experience requirements.   
c.  DCS/HR-026 – Employment Application form must be filled out completely, printed, signed, and brought for the interview.
